Requesting Tax Information | Instructions on how to request tax documents from the IRS that are required for the verification process. |

Online Credit Hour Application | An application for online and graduate students to submit how many credit hours they will be enrolled in per semester. A new form will be completed for each academic year. This must be completed by all online students. |
Return of Title IV Funds Policy | The federal aid return policy if a student is to withdraw or drop courses. |
Satisfactory Academic Progress Appeal Form | The appeal to have aid potentially reinstated once aid is terminated based on not meeting satisfactory academic progress. |
Satisfactory Academic Progress Policy | The policy that explains the academic requirements to stay in good standing to be eligible to receive federal aid and the process if a student loses that eligibility. |
Petition for Independent Status | An application for students who are dependent status per the FAFSA qualifications that have extenuating circumstances that prevent them from providing their parental information. |
Special Circumstance Request form | An application to report if you or your family had a significant change in your financial situation that is not accurately represented on your FAFSA. |
Undergrad Parent Plus Loan Application | A loan application through the federal government taken out in name of the parent of a dependent undergraduate student to be used towards the student’s balance. |
Graduate Plus Loan Application | An application for masters or PhD students for the federal graduate PLUS loan. |
Options for Paying for College | Information about various ways to cover a balance. |
Title IV Student Authorization Form | A form that authorizes Tiffin University to use your federal aid funds towards your student account balance. |
Revolving Credit Agreement | An agreement between the student and Tiffin University that authorizes the student to charge books from the bookstore to their student account. |
Tiffin University Refund Check Policy | An explanation of how refunds are processed if there is a credit on your student account. |
Electronic Refund Instructions | Instructions on how to sign up to have a refund direct deposited to your bank account if you have a credit on your student account. Refunds of parent plus loans are sent to the parent in a check. |
Master Promissory Note | Is a legal document in which you promise to repay your loan(s) and any accrued interest and fees to the U.S. Department of Education. It also explains the terms and conditions of your loan(s). |
Entrance Counseling | Students that are taking a Federal Student Loan must complete Entrance Counseling. |
Annual Student Loan Acknowledgement Form | If it is your fist time accepting a federal student loan, you must acknowledge that you understand your responsibility to repay your loan. If you have existing federal student loans, you must acknowledge that you understand how much you owe and how much more you can borrow. This must be done annually. |
Exit Counseling | Is required before you withdraw, graduate, or drop below half-time attendance even if you plan to transfer to another school. |
