Student organization Forms

Planning to serve food at your on-campus event? This form is essential. If AVI is catering, simply specify the types of food needed. If you’re bringing in food from an outside source, you’ll need AVI’s approval. Be sure to include the food types and costs. This completed form must be attached to your event registration.


Reflecting on your event is key to future success. After your event, use this form to assess what went well and identify areas for improvement. Your feedback helps make every event better.


Planning a fundraiser for your campus group? Get approval from the Advancement office with this form. Outline your fundraising goals, intended use of the funds and where the activity will take place.


Thinking of starting a new organization on campus? This form is your first step. It covers the group’s purpose, advisor details and other important aspects needed for official recognition.


Need financial support from Student Government for your event? Submit this form at least two weeks in advance. Detail your funding needs, event plans, and how it will benefit our community. Your request will be reviewed by Student Government.


Every new campus organization needs a constitution. Use this template as a guide to create a foundation for your group. Once completed, submit your constitution for Student Government approval.


Want to register an event on campus? This form must be completed. It goes over what is necessary for the event, and what the details of the event are. It must be completed at least a week in advance. 


Organization Membership

In order for a group to become a University recognized organization, the group must meet the following requirements.

  1. The group must have a full-time Tiffin University faculty or staff member serve as an advisor.
  2. The group must have its constitution approved by the Director of Student Activities and Student Government.
  3. The group must successfully complete a one-semester probation period.
  4. The group must follow all University procedures and policies.

Individuals

In order for an individual to participate in a club or organization, they must be currently enrolled at Tiffin University and must be in good academic and social standing with the University.
